housing Case manager
The Housing Case Manager is responsible for supporting individuals and families experiencing homelessness or housing instability through assessment, service planning, housing navigation, advocacy, and coordinated referrals. The position requires strong documentation practices, professional judgment, and collaboration with community partners to promote participant stability and long-term housing success.
